DECLARATION OF THE SUPPORTING MEMBER
I hereby request to conclude a contract with me for the provision of services by Association of Psychologists and Psychotherapists for Society No. KRS 0000860575 .
INFORMATION CLAUSE:
In accordance with Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of individuals with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46 / EC (general regulation on the protection of data) "GDPR", we would like to inform you that the administrator of personal data is the Association of Psychologists and Psychotherapists for Society, KRS number 0000860575, based in Warsaw, ul. Żurawia 6/12 lok. 766.
Personal data will be processed on the basis of your consent (Article 6 (1) (a) of the GDPR) in order to:
- implementation of the statutory tasks of the PDS Association,
- supporting membership in the PDS Association,
- fulfillment of the legal obligation incumbent on the Association (this applies to cases of providing data at the request of competent authorities or courts).
Personal data will not be shared with other recipients. The recipients of personal data may be IT solution providers (including the website provider www.wix.com and its subsidiaries), other entities if their participation is necessary to achieve the purposes of processing. In justified cases, personal data may be transferred to entities based outside the European Economic Area (EEA).
Your personal data will be processed for the duration of the membership of the PDS Association, not longer than for a period of 5 years. The processing of your personal data is necessary for the purposes of the legitimate interests pursued by the Administrator in connection with the statutory activity and is carried out pursuant to Article 6 par. 1 lit. f) GDPR.
You have the right to:
· Resignation from membership,
Requests for access to personal data being processed,
Rectify them,
Delete data,
Processing restrictions,
· Withdraw your consent at any time.
You have the right to object to the processing of data and to lodge a complaint with the President of the Personal Data Protection Office.
Providing personal data is necessary for admission to membership of the Association. Providing other data by you is voluntary.
